How Professionals Can Access CREATE

Referrals are accepted from any member of the CMHT's, who carries the Care Co-ordinator role for the individual referred. This may be a CPN, Social Worker, Occupational Therapist or Medical Officer. Referrals may be made for individuals at either the Standard or Enhanced level of the Care Programme Approach.

Referrals will only be accepted on CREATE referral forms and must be accompanied by a care plan & risk assessment. All sections highlighted in red must be completed or the referral will be returned.

One referral only is needed for all services.
