Providing Day Services for adults suffering mental health problems in the north of the City and County of Swansea
This is the area covered by the Comunity Mental Health Teams
for areas 3 and 4. As of April 1999 responsibility for the service was taken
over by Cwmbwrla Day Centre.
The service consists of 10 groups throughout the week, ranging from social
groups in Gorseinon, Llansamlet and Toronto Place in Caereithin. Activities
include rambling, yoga, health & fitness and arts and crafts.
The service accesses community facilities such as Community Centres and Penlan
Leisure Centre. We also arrange regular social outings.
The service was originally set up for service users who could not access the
service provided by Cwmbwrla Day Centre and to meet the need for a peripatetic
service.
An Overview of what is on offer to Care Managers working with new referrals
New Referrals
New referrals
are to be sent to CREATE at Cwmbwrla Day Centre.
Referrals must include a risk
assessment and a copy of the care plan.
Pre-referral visits by care
managers and prospective attendees are welcomed.
Upon receipt of referral a
staff member will contact the care manager to arrange a start date.
Due to a limited capacity
on the minibus, transport requirements will be discussed with the care manager
and service user prior to commencement. Bus tickets can be refunded.
Change in Circumstances
Care managers must inform the service of any change in
circumstances e.g. If service users are going into hospital, Llanfair, on holiday
or are not attending for any other reason please contact us so we can adjust
the pick up list and save unnecessary trips. If the bus is turned away twice
we will contact the care manager regarding whether to continue the service
for their client.
Please also notify us if there is a change of care manager.
Due to the limited staffing available to the service, we are unable to accept
service users should their mental health deteriorate to a state where it adversely
affects the majority of the group. We are also unable to accept service users
if they are under the influence of alcohol or illegal drugs.
