How Service Users Access CREATE
There are two ways you can access CREATE services.
Some CREATE member organisations have an 'Open Door Referral Policy or are Open Access' You can attend that organisation when you want to, without needing to ask anyone first.
These are:
• Connect
• Hafal
• Dragon Arts Centre
• Mind
Other CREATE member organisation are only by 'Referral via CREATE'. To attend these you need to speak to your Care Co-ordinator who may be a Community Psychiatric Nurse, Social Worker, Occuaptional Therapist, or Medical Officer.
These are:
• Cwmbwrla Day Services
• Fforest Industries
• Woodwork for Day Patients at Cefn Coed Hospital
